The asleep-awake-asleep surgery is a growing anesthetic technique, which is used in craniotomy procedures in order to allow the surgeon to explore the epileptogenic areas of the brain with the purpose of removing them effectively and safely, protecting the patient from possible complications that otherwise could not be evidenced in the transoperative period. In the present work we present two clinical cases of patients with epilepsy in whom the asleep-awake-asleep (DDD) technique was performed. By means of clinical evaluation and MRI with spectroscopy and contrasted MRI, the epileptic foci were located. Both patients were operated by awake-sleep-awake craniotomy, with a short postoperative stay, without complications in the transoperative and postoperative period. Both showed a significant reduction in seizure frequency three months after the surgical procedure.
